As some ladies screening appointments are overdue, please note we are currently offering evening appointments at The Breast Care Centre, The Captain Sir Tom Moore Building, Halton and the Burney Breast Unit, St Helens Hospital




This appointment request form is ONLY for:

a) Women who have received an invitation to attend for screening under the NHS Breast Screening Programme and wish to arrange or rearrange an appointment. 

b) Women who have missed an NHS Breast Screening appointment and would like to request a new appointment. 


IMPORTANT: Women under the age of 50 will not be sent an appointment if they submit this request form. If you are concerned about any breast symptoms please make an appointment to see your GP as soon as possible and your GP will refer you to the appropriate clinic if necessary.


Please note that whilst we try to offer a wide range of dates and times we do not screen on every day at each location. We will try to book you an appointment as near as possible to the dates and times requested

Please note that you may be invited to attend an appointment at our new state of the art Breast Care Centre at the Captain Sir Tom Moore Building next to Halton Hospital in Runcorn. Situated at the rear of Halton Hospital, this brand new purpose built facility has ample parking and easily accessible from the M56 and other local routes in the area. If this location is not convenient and you would prefer to attend another one of our screening sites, please email or call the team on 01925 275500.

The Captain Sir Tom Moore Building (formerly known as Cheshire and Merseyside Treatment Centre) at the rear of Halton General Hospital is home to our new state of the art Breast Care Centre which was opened in July 2021. The centre has 2 new mammography rooms, 2 ultrasound clinic rooms, consultation and counselling rooms, which will ensure that patients receive the best specialist care in a dedicated and supportive setting. This new unit will ensure our patients receive first class specialist care in a fantastic new environment. Breast Screening and Assessment clinics, along with Rapid Access Symptomatic Clinics, will operate from these new improved facilities, with benefits for both patients and staff.
